Audition Form Istructions
Completed by the Choir Director/Music Teacher

1.

Click on the KJHC Audition Form link and an Excel package will open.
By answering the questions on the Audition Form page, the Invoice, Soprano, Alto, Tenor, Baritone, and Spreadsheet Form fields will automatically be populated.

After opening the link, click on the tab “Audition Form” (bottom left side of page) to open the correct page.

Save this form. Click on “File” in toolbar and then select “Save As.” Re-name the workbook as “KJHC Audition for (your School)”. Click “Save.”
4.
Type your information in the yellow fields.
5.
Answer all questions concerning school and director(s) at the top of the page.
6.
Student information is divided according to vocal part. DO NOT COMBINE VOCAL PARTS IN THE SAME SECTION!!!!
7.
Type the names in the order that the students are recorded on the audition tapes/CDs.
8.
Information required for each student:
- First and last name as will appear in the concert program and on   certificate
- Grade level
- Vocal part
- Shirt size in youth or adult size
  (YS, YM, YL, AS, AM, AL, AXL, AXXL, etc)
- Concert height in inches
  (measure students wearing their concert shoes)

To print each of the six forms click on each of the tabs at the bottom left of page. You must open and print each page separately.
10.
Print 3 copies of the Invoice Form: 1 for KMEA office, 1 for KJHC Coordinator, and 1 for your files.
11.
Print 2 copies each of the Soprano, Alto, Tenor, and Baritone Files: 1 of each copy to the KJHC Coordinator and 1 of each for your files. (Print only the vocal forms for which you have students auditioning.)
12.
DO NOT PRINT THE PAGE LABELLED "SPREADSHEET".

Save and Close File
14.
Mail the Invoice Form with check for total audition fees to the KMEA Offices.
15.
Mail the Invoice, Soprano, Alto, Tenor, and Baritone Forms to the KJHC Coordinator with the audition tapes/CDs. (Mail only the vocal forms for which you have students auditioning.)
16.
Email the entire saved package as an attachment to the KJHC Coordinator.

All hard and emailed copies of forms must arrive at the KMEA office and the KJHC Coordinator home by 8:00 pm, November 1, 2008. No Expceptions!

All packages should be sent without recipient signature required. Wave any signature requirements. Packages will be refused if signature is required. No Exceptions!
